WHAT BENEFITS DOES MY PERSONAL ACCIDENT INSURANCE POLICY (WHICH IS UNDERWRITTEN BY THE HOLLARD INSURANCE COMPANY) COVER?

If you are involved in any accident (a fortuitous unexpected event occurring at an identifiable place and time), you will have the following cover: -Accidental Death: R10,000 per insured person -Emergency Medical Expenses: R30,000 per insured person -Emergency Medical Transportation: R20,000 road transport ;R100,000 air transport -Repatriation to Domicile in RSA: R20,000 per insured person -Memorandum: -Pre-existing medical conditions excluded R 500 excess applicable for Emergency Medical Benefit -Celtis Financial Services (014) 533 1021 or email stefan@celtis.co.za

WHAT DOES MEDCLEAR GROUP SCHEME DO FOR ME?

• National 24/7 Emergency call center who will: o Determine your exact location if you have pressed the Panic button on the Medclear 24/7 Panic Mobile Application o Determine the nature and extent of your emergency o Dispatch a road or air ambulance to you (depending on the severity of the injuries and assessment) or direct you to the closest hospital or emergency medical facility o Make all the arrangements regarding invoices and payments with the ambulance service o Make all the arrangements regarding opening a file, invoices and payments at the closest hospital or emergency medical facility as well as admission (if necessary) • Claims management team who will: o Follow-up with the ambulance service and hospital (service providers) regarding treatment, claim forms, invoices and reports required o Manage all invoices, including feed-back to you, your medical aid and the service providers o Liaising with Hollard Insurance Company to ensure that the claims are managed and paid timeously

IS THIS A MEDICAL AID?

• No, this is not a medical scheme and the cover is not the same as that of a medical scheme. • This policy is not a substitute for medical scheme membership. This is Emergency Medical cover.

WHAT DO I NEED TO DO IN AN EMERGENCY?

• In case of an Emergency inform our National 24/7 Emergency Call Centre immediately by: o Pressing your Panic button on the Medclear 24/7 Panic mobile application or o Calling us on 087 980 4357 or o If you do not have airtime, enter the *134*7714# USSD code from your phone to log an emergency and then our National Call Centre will phone you back o This will serve as the registration of your claim o Medclear will inform Celtis Financial Services and The Hollard insurance company of your claim

ONCE I HAVE REGISTERED A CLAIM, WHAT DO I NEED TO DO?

• Go to the medical facility as arranged by the Call Centre Agent • Report to the contact person as provided by the Call Centre Agent • You need to provide the following information to the Call Centre Agent and/or the medical facility: o If you have a medical aid you will need to provide medical aid details o If you do not have a medical aid, you will need to complete an affidavit stating this and email this to admin@medclear.co.za. A copy of an affidavit is attached for your convenience
